2/20/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted Ashiyah upon arrival to the home setting. Upon arrival to the home setting, Ashiyah appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHP-C informed Ashiyah that this session was going to focus on continuing to strengthen her communication skills and include her mother in the session. 2. Behavior: Ashiyah contributed to the session and did not need any redirection throughout the session as she participated fully in session. Ashiyah continues to report struggling with being able to communicate with her mother in an effective manner due to her mother being sick at times and her not feelings as though there is a connection between the two. 1. Intervention: QMHP-C began the session by inquiring about Ashiyah’s current thoughts, feelings and emotions. QMHP-C engaged Ashiyah and her mother in an activity purposed to assist Ashiyah with verbalizing and expressing emotions. QMHP-C explained to both Ashiyah and her mother that this activity required Ashiyah’s to shoot a ball into the garbage can. Made shots required Ashiyah to discuss positive/happy experiences. Missed shots required Ashiyah to discuss negative/sad experiences. QMHP-C processed the activity and encouraged Ashiyah to verbalize their emotions rather than act out aggressively. 2. Intervention: QMHP-C revisited appropriate communications skills such as listening to the person who is talking, answering questions, and giving eye contact. QMHP-C revisited communications guide with family. QMHP-C engaged Ashiyah and her family in a role play exercise which instructed Ashiyah to demonstrate effective communication skills. QMHP-C engaged the family in an activity called “Smiley Face” purposed to illicit positive memories. QMHP-C encouraged Ashiyah to think about positive memories as a technique to manage depression when she doesn’t have control over certain things in her life. 1. Response: Ashiyah appeared to be receptive to QMHP-Cs’ interventions and services that were provided to her during the session. Ashiyah and Ms. Hodge participated in the therapeutic activity and were enthused evidenced by their laughing. Ashiyah demonstrated effective listening when her mother were verbalizing their experiences and during the role play exercise. She contributed to the discussion on communication skills by identifying communication skills. Ashiyah participated in the Smiley Face activity by describing positive memories. 2. Response: Ms. Hodge discussed with QMHP-C that she just wants Ashiyah to know that she is always there for her and that she can come and talk to her about anything. As she has mentioned in the past, she does not want her child to become pregnant at a young age nor does she want her daughter to be another child that has been caught in some cross fire and she is then burying her child. Ashiyah listened to her mother and indicated to her mother, “I do try to communicate with you ma but you never listen to me and you are always telling me I cannot do certain things and I get mad”.
